Job Description

As a Clinical Pharmacist, you will play a key role in ensuring medication safety and regulatory compliance within hospital settings. You will work closely with medical and nursing staff to ensure the accurate prescription, dispensing, and administration of medications.

Key Responsibilities:

1. Conduct medication reviews and clinical audits before administration.

2. Handle transcription/indenting of drug and non-drug orders.

3. Track and receive medications from the pharmacy.

4. Audit in-patient orders for:

   • Drug name, strength, formulation

   • Dosage and therapy duration

   • Therapeutic duplication and potential drug interactions

5. Check Medication Administration Records and rectify any errors.

6. Review patient records in wards/ICUs and consult with the Clinical Pharmacologist for clarifications.

7. Manage non-formulary drug requests and suggest substitutions from the hospital formulary.

8. Regularly inspect:

   • Impress stock

   • Crash carts

   • Disaster kits and ambulance supplies

9. Document and report restricted antibiotic usage to the Clinical Pharmacologist.

10. Monitor and report Adverse Drug Reactions (ADRs).

11. Act as a drug information resource for healthcare staff.
